张柏芝饰演周璇，最先备受诟病的便是外形上的差异。因为周璇是个柔美丰腴的美人，而张柏芝则过于骨感，且张柏芝眉毛太浓太霸气。据悉，为了尽量靠近原型，这次张柏芝“周璇”造型由王家卫的御用造型师张叔平设计的：把原本粗粗的眉毛修成了柳叶形，以符合“周璇”的神韵；为张柏芝设计了“月明歌舞厅时期”、“当红时期”和“成名后期”三种造型，张柏芝在剧中共有48套服装，这48套服装包括旗袍、学生装、小西装、洋装等，专门请人在香港制作，服装费达200万元。
张柏芝对自己也很自信，“‘周璇’这个角色非常好，非常大方，我演得很舒服……还没开拍之前我做了很多功课。我们都是做演员的，看到她写的东西，就感觉自己好像活在那个时代。她那个时候受到很多压力，遇到很多不开心的事情，虽然周璇是上世纪三四十年代的人，但跟我们没有什么差别”。
不过即使如此，周璇的儿子周伟依然不敢恭维：“张柏芝在里面换了很多套造型，一套套都很漂亮，可是依然不像我妈妈。”
